YOUR HEALTH ON YOUR TERMS

Home telehealth is a rapidly expanding service that provides homeowners with the ability to stay connected with their healthcare providers with an easy and inexpensive method. Telehealth also provides an added level of security and efficiency for those homeowners with chronic conditions, and those in need of routine monitoring.

Home Telehealth is the remote patient monitoring (RPM) between a health care provider and a patient at their place of residence. The focus of Citadel Telehealth is to help home health agencies monitor their patients, and for homeonwers to have quick and easy access to their healthcare providers. Citadel Group can install systems that will assist the home health agencies in monitoring for any behavior changes in their patients, helping the patient with self management of conditions, and in collecting data on their vital signs 24/7.

PARTNERS YOU CAN TRUST

Citadel Group has partnered with Acadian Telehealth Monitoring, and uses the Viterion 100 & 500 monitoring devices. These cutting-edge systems have been developed through a unique initiative of Bayer HealthCare LLC and Panasonic Company.

Viterion TeleHealthcare’s products use highly reliable store-and-forward technology to relay vital sign measurements and personalized questions or advice, as well as digital video technology and Web access. The network is managed by Viterion TeleHealthcare, with stringent measures to ensure the safety and security of patient information, providing a turnkey solution for providers.

We use highly encrypted resources to meet HIPAA’s high security and privacy standards. All data exchanged between the monitoring station, the home health agency, and the patient is secured using state-of-the-art encryption technology. For added security, multiple firewalls and authentications are utilized to protect patients’ personal information.

In some cases, Telehealth
can replace a physical visit,
but it can also provide real-time information that assists healthcare providers in delivering a greater level of service.

Citadel Group Telehealth enables the healthcare professional to conduct routine monitoring, enables greater access to the patient, and can also allow providers to be more responsive to patients with critical needs. With monitoring by Acadian’s highly trained staff, should a patient become at risk, the home agency provider will be immediately notified and the patient will receive prompt response. Telehealth technology makes each visit to the patient more cost effective, while providing quicker access and intervention to those patients with at-risk needs.
